Every entry into the Best In Skin Awards needs your client's consent — and that consent needs to cover more than permission to take a photograph.
This form does the explaining for you. Written in plain language for your client, it sets out what's being shared, who sees it during judging, and what happens if your entry is named a finalist or an award winner. It captures their signature, and includes a section for a parent or legal guardian where your client is under 18.

Keep the signed original in your own records. Don't send it with your entry. Best In Skin Awards may request a copy at any time, and entries cannot be accepted or awarded without it.
A note on saying no
The form makes clear to your client that they're free to decline, and that their care isn't affected either way. That's deliberate. Consent your client understood is the only consent worth having — and it protects you as much as it protects them.
